York,PA LTCF Application and instructions
Could not find this on PAFOA, and looked it up for my brother-in-law so I figured I'd post it here.
Direct link to application for a printable york county LTCF in PDF format: http://ycwebserver.york-county.org/s...007%20FORM.pdf This application appears to be a Pennsylvania form, so it may be used in other or all counties.
