Does this form need to be sent with Form 4 for machineguns, suppressors, or both?

ONE copy of the form 5330.20 - "Certificate of Compliance With 18 U.S.C. 922(g)(5)(B) " is required with all Form 1s to make a firearm, and all Form 4s for any NFA item requiring transfer.

its the stupidest form ever, YOU are required to certify that YOU are a citizen.

they even have the balls to post a "Paperwork Reduction Act " notice on the back

I had a form 1 kicked back because I signed the top line, and not the second line with the same name. So I signed it sent it back and within three weeks I had my approval to make a NFA weapon. Crazy Law, but then most are!

That happened to somebody I know (not me) who's dealer is northwest of Allentown.

just to get this straight as i'll be filling mine out tonight and this form is so simple and stupid it scares me like i'm missing something, oh wait thats ALL of these forms.

line 1 and 2 i print my name. line 3 says what is your country of citizenship, which is united states obviously but then it says "country name(list additional county(ies) below if applicable)" since I only am a US citizen do i leave this blank? then just sign and date the bottom

Hey XD - I have become quite the expert on this form .. Don't ask menhow

As per my recent letter and kickback of this form from our friends at the ATF:

Line 1 is your name
Line 2 is you certifying that yourself as a US Citizen - FUN!
Line 3 USA is fine - NO need for anything on the second line. (I'm guessing this line is for folks with Dual-Citizenship or something)
Line 7 Sign and Date

Line 2 seems to be the clincher for many folks.. some dealers aren't even sure on it. As per my examiner, that was the only issue with my form which was filled out as I explained above.

Hi all, I'm new here and since I've had a class III weapon for a little over two years ago, I'm still new to it all.
Regarding the subject of this revised form, here's a question: A couple of years ago when I purchased my first suppressor (integral on a Ruger 77/44), I filled out an ATF form 5330.20. It was the one with the square blocks that you fill in one letter of your name in each box. Everything went without a hitch, got my stamp in 59 days.
In March of this year, I purchased a Gemtech can, got everything I needed and sent it off, to the old Chicago address (my dealer is a very casual seller of class III items and didn't know about the address change). So I get it back, get the new Georgia address and send it promptly. Now, seeing this thread about a revised form, I'm wondering if the first form (with the square blocks) will work. It has the same ATF form number (5330.20), but looks different. I went to the ATF website and downloaded and filled it out. It's basically the same form, only without the square blocks. Any ideas?
